Quitline Fax Referral Form - Patient Referral/Consent Form for the Kentucky Tobacco Quitline.  As a healthcare provider, simply fill out the form for your patients/clients, have your patient sign the form and fax it to the number of the form.  A trained counselor from the Kentucky Tobacco Quitline will contact your patient/client to discuss their options for tobacco cessation.  This is a FREE service.
